When you run a pet sitting business, it is important to use a client services agreement form. This form
ensures that I get all of the information I need from my clients & helps them understand the services I provide &
the fees charged for those services. By signing an agreement, you formalize the relationship & ensure we have a shared
understanding of the services I’ll be providing.
The client services agreement will provide general information
about you including name, address and contact phone numbers. It includes access information for the home such as alarm or
security codes and the location of additional keys to the home & finally, instructions about the return of your keys.
The charges for the services I’m offering are on the agreement. Signing the document indicates acceptance
of my rates.
Finally, I’ll need specific information about your current needs. Specifically, get the dates the
service is needed, the number of visits per day and any special requirements for that visit.
In addition to the client services agreement, fill out the Pet
Facts sheets with information about each pet & complete the Veterinary release so that you can seek care if an animal becomes ill or injured while under
my care.
Neighbor Notification: This optional form will
let your neighbor know that Prairie Sky Pet Sitters will be coming to care for
you Pets. It will provide my name & contact number, my services & a brief discription of my cars. It's so
embarassing to have a neighbor call 9-1-1 when they see a stranger at your home.
I will email or fax you the forms you need to fill out. Print copies of the forms, complete the
requested information & we can go over them on my initial visit. |
